At least one must be true, and in every case you must not be eligible for TRICARE (the two programs are mutually exclusive):
- You’re the spouse or dependent child of a veteran rated Permanent & Total (P&T) — either a 100% schedular rating with no future exam scheduled, or a TDIU determination the VA has classified as permanent.
- You’re the surviving spouse or dependent child of a veteran who died from a service-connected disability.
- You’re the surviving spouse or dependent child of a veteran who was rated P&T at the time of death, regardless of cause of death.
You generally do NOT qualify if:
- The veteran’s rating is below 100%, or is 100% but not designated P&T.
- You or the veteran are eligible for TRICARE.
- You’re a surviving spouse who remarried before age 55 and that marriage hasn’t since ended.
- You’re a dependent child over 18, not a full-time student, and weren’t permanently disabled before age 18.

VA Form 10-10d, “Application for CHAMPVA Benefits,” starts every CHAMPVA application. Download it and submit it one of three ways:

Proof of Marriage (for Spouses)
Submit a copy of one of the following to help process your application faster:

Remarried before age 55 and that marriage has since ended? Also include a divorce decree, annulment decree, or death certificate to reinstate eligibility.
Proof of Birth or Parentage (for Children)
- Birth certificate for each dependent child
- Adoption papers, if the child was adopted
- Proof of marriage between the veteran and the stepchild’s other parent, if the child is the veteran’s stepchild
- School certification letter, if the child is between 18 and 23 and enrolled full-time (this must be renewed annually)
- A disability rating letter (sometimes called a “helpless child” determination) if the child became permanently unable to support themselves before age 18
Medicare Documentation (When It Applies)
If you or your dependent are 65+ or otherwise Medicare-eligible, you must have Medicare Part A and B (or Medicare Advantage) to keep CHAMPVA. Submit, with VA Form 10-7959c:
- A copy of the front and back of the Medicare card, or
- A “notice of disallowance” from the Social Security Administration if you’re 65+ and not enrolled in Medicare
- A copy of the Medicare Part D card, if applicable
Other Health Insurance Documentation
Other health insurance besides Medicare? Submit a copy of the card, front and back, with VA Form 10-7959c. CHAMPVA becomes the secondary payer whenever another plan is in place.
Social Security Information
Every applicant needs a valid SSN on file. For a newborn, get the child’s SSN first, then add them as a dependent through your nearest VA regional office — VA can’t pay claims for an unenrolled child.
